A Kaniksu Community Health RN/LPN Care Manager is not just responsible for providing excellent customer service. They are committed to providing an excellent patient experience while performing essential clinical or administrative work in a fast-paced, productive environment. YOU ARE an essential team member charged with assisting patients in managing their care at Kaniksu Community Health.
Under the direction of the KCH Director of Nursing, KCH Charge Nurse and Medical Staff Providers and as a member of the Kaniksu Community Health Patient Centered Medical Home (PCMH) Team, a typical day might include of a RN/LPN Care Manager might include;
- Oversee clinical care of other personnel including LPN’s, CNA’s and MA’s
- Has a commitment to creating a positive team environment and being a good team member of his/her PCMH Team
- Assesses, plans, directs and evaluates comprehensive nursing care as determined by the patients age related to physical, psychosocial, and cultural needs in accordance with profession practice standards, policies and procedures of KCH and the State of Idaho scope of practice
- Provides case management by being a patient advocate, resource and contact person for patients
- Reviews patients’ medical records for completeness and updates prior to the Patients’ visit – with a focus on health maintenance and chronic disease management clinical protocols
- Provides basic education information to patients and families, focusing on self-care appropriate to their age and identified learning considerations and needs
- Performs assessment data collection and documents subjective data and objective data
- Reviews basic health history information for patients; focusing on environment, self-care and clinical discharge planning

Experience Needed to Land this Gig:
- Minimum Associates Degree in Nursing; BA in Nursing preferred. Current license to practice as a RN or LPN in the State of Idaho
- Minimum 1-3 years’ experience in ambulatory care. Experience in a community health center is preferred by not required
- Capable of independent nursing assessment decisions and nursing management in a primary care setting
- Able to provide health care in a sensitive, non-judgmental manner to persons of all economic levels and cultural backgrounds
- Demonstrate accuracy, proficiency and efficiency with computer skills for accurate data entry; effective written and oral communication
- Consistent demonstration of team approach to patient care

- 40 hours/week
- 4x10s – Tuesday to Friday
