A Kaniksu Community Health CMA/CNA - Referral Coordinator is not just responsible for providing excellent customer service. They are committed to providing an excellent patient experience while performing essential clinical or administrative work in a fast-paced, productive environment. YOU ARE an essential team member charged with assisting patients in managing their care at Kaniksu Community Health.

Under the direction of the KCH Director of Nursing, KCH Charge Nurse and Medical Staff Providers and as a member of the Kaniksu Community Health Patient Centered Medical Home (PCMH) Team, a typical day might include of a CMA/CNA - Referral Coordinator might include;

• Documentation and implementation of policies and procedures defining the referral process at KCH for all clinic locations.

• Working directly with insurance payers to pre-authorize outgoing specialist referrals.

• Maintaining an up-to-date registry of available specialists in the area; work directly with KCH providers to keep an up- to- date log of all internal referral possibilities; make recommendations to utilize internal services when/where appropriate.

• Serve as a resource to referring providers and payers on the specialist / organization most suited to fulfill the referral request.

• Generate records within NextGen with all pertinent referral information, deliver the referral letter via fax and/or via the Share Portal and track that document through all stages of the referral to completion.

• Update the patient’s medical health record (including diagnoses, health histories, and/or other pertinent health management information) if/when applicable

• Perform outreach to referred specialists, to facilitate and coordinate continuity of care

• Review and close care gaps as appropriate with referral processing

• Manage tracking logs to ensure timely follow-up and completion on all created referrals.

• Assist with quarterly and annual referral tracking, including evaluation of measurable outcomes in the referral department

• Facilitate communication between the provider care teams, the referred specialist, and the patient if/when applicable to coordinate & complete open referrals

• Work closely with KCH providers, nursing staff, and office personnel to capture and document co-management relationships within NextGen; works closely with the referral coordination team, to ensure that pertinent patient data is being transferred as per the co-management agreement.

• Monitor Meditech to ensure that all KCH patient hospital admissions are captured; generates patient history documents and transfers that data to the hospital in a timely fashion to enable a high level of continuity in their care.

Experience Needed to Land this Gig:

• High school diploma or general education degree (GED); or equivalent combination of education and experience.

• 2+ years’ experience in a medical environment with knowledge of medical terminology required (MA, CNA licensing preferred).

• Computer skills to include proficiency in word processing; Excel (building, manipulating spreadsheets), strong technical aptitude with the ability to learn new programs quickly; previous experience using an Electronic Health Record preferred, strong preference for NextGen knowledge.

• Must be highly organized and detail oriented.

• Must be process minded and able to follow precise directions;

• Advanced, extensive, and documented experience in general office administration and office functions;

• Excellent oral and written communication skills.

• Ability to maintain corporate confidentiality and strict HIPPA compliance.

• The ability to work in the absence of direct supervision.

• Excellent time management with the ability to work under conditions of constant interruption while remaining on task.
