A Kaniksu Community Health RN/LPN – Family Practice is not just responsible for providing excellent customer service. They are committed to providing an excellent patient experience while performing essential clinical or administrative work in a fast-paced, productive environment. YOU ARE an essential team member charged with assisting patients in managing their care at Kaniksu Community Health.

Under the direction of the KCH Director of Nursing, KCH Charge Nurse and Medical Staff Providers and as a member of the Kaniksu Community Health Patient Centered Medical Home (PCMH) Team, a typical day of a RN/LPN – Family Practice might include;

  • Works cooperatively with the KCH Medical Providers and personnel to carry out the goals and objectives of KCH according to the established policies and procedures.
  • Reviews patient medical records for completeness, including the provider PAQ and updates the medical record as appropriate.
  • Assist the team as needed to provide treatment and/or medical procedures
  • Perform nursing Flip Visits to increase patient access to KCH, including but not limited to rooming, reason for visit, document chief complaint, and obtain pertinent patient history.
  • Takes and records patient vital signs, including but not limited to blood Pressure, pulse rate, height and weight, etc.
  • Perform screening/testing under the direction of the Medical Providers.
  • Performs routine patient procedures (routine lab work, etc. as per Nursing/Provider direction or as indicated by protocols. Assists Provider with patient care tasks and procedures i.e. Pap tests/pelvic exams, surgical procedures, and dressings.
  • Prepares patients for diagnostic tests; collects and labels specimens, completes lab tests allowable by CLIA certificate.
  • Administers medications, gives injections which may include administration and documentation of medical administration consistent with his/her knowledge of pharmacology in accordance with health center policy.
  • Calls in prescriptions, following written Medical Provider orders.
  • Fills out laboratory and diagnostic testing requests and makes patient appointments following written Medical Provider orders.
  • Assists multiple providers on occasion with established patient care protocols and works collaboratively for the coordination of quality health care as part of the Medical Home Team.
  • Maintains a clean, safe, orderly work area and washes, wraps and sterilizes instruments as required.
  • Adheres to infection control policies.
  • Prepares patient record information for physician referrals and communicates significant information to other health care providers as required.
  • Participates in preventive health teaching and education pertinent to procedures being conducted as delegated by the Provider.
  • Provides guidance to non-clinical staff regarding clinical issues (i.e. phone triage).
  • Maintains IRIS inventory and tracking, including but not limited to participating and conducting annual IRIS site visits.
  • Maintains compliance with PharmaWatch and monitor for temperature excursions, including submitting temperature incident reports to our IIP.
  • Logs patient information in respective data banks, i.e. IRIS Idaho State Immunization.
  • Responsible for attending and actively participating in PCMH Team Meetings, Staff Meetings, In-Service and Educational Programs.
  • Assists with orientation of rotating clinical personnel.
  • Committed to creating a positive team environment and being a good team member. Works with the PCMH Team to assure that all KCH patients are given top priority both personally and professionally by all medical support staff and that patients and providers needs are met through appropriate systems and the highest level of patient services.
  • Responsible for relating well to people from diverse ethnic and cultural backgrounds and for having empathy for patients with demonstrated ability to treat them with dignity and respect.
  • Responsible for providing accurate and thorough work product with attention to detail; maintains accurate documentation on patient’s medical record according to established guidelines for electronic medical record.
  • Responsible for supporting compliance with federal, state and other agency requirements. Strict adherence to confidentiality of all information related to KCH patients and organizational information.
  • Responsible for managing priorities and workload, time management to ensure effective workflow in a fast paced clinic environment.
  • Responsible for adherence to policies and procedures and supporting standardization and integration across the KCH organization.
  • Maintains knowledge of universal precautions and blood borne pathogens.
  • KCH Team Member is expected to assist with clinic training to ensure compliance with OSHA and Infection Control requirements.
  • Willingness to work flexible hours in order to meet patient care needs/demands.
  • Performs related work as required.

Experience Needed to Land this Gig:

  • Minimum Associate Degree in Licensed Practical Nursing or Registered Nurse equivalent required.
  • 2+ years similar work or internship required.
  • Experience in a community health center, public health or a community hospital is a plus but not required.
  • Must be licensed to practice as an LPN or RN in the State of Idaho
  • Possess cognitive skills necessary to understanding medical instructions and identifying problems and offering solutions.
  • Must demonstrate accuracy, proficiency and efficiency with computer skills for data entry.
  • Demonstrate effective people skills with the ability to communicate well with patients and families utilizing tact and professionalism
  • Consistent demonstration of team approach to patient care.
