Under the direction of the KCH Director Clinical Operations, and as a member of the KCH Patient Centered Medical Home (PCMH) Team, CNA Medical Records Clerk serves in a medical facility handling patients’ files and making sure their information is transferred and stored properly. The position is responsible for collecting patients’ personal information, like their medical history and demographics. Medical Record Clerks have a strong knowledge of laws and regulations in order to legally issue medical files to the allotted people and agencies. Medical Records Clerks must also have advanced understanding of the KCH policies and procedures to properly keep accurate records. They must also make sure patients’ paperwork, reports and charts are all completed correctly and efficiently.

Position Qualifications:

• Graduate of an accredited Nursing Assistant program or a Certified Medical Assistant certification

• Required to have prior experience in a medical facility and expertise

• 1-2 years of experience in a medical office setting

• Prefer to have certifications as a Registered Health Information Technician (RHIT) or the Certified Electronic Health Records Specialist

• Experience in a community health center, public health or a community hospital is a plus but not required.

• Knowledge of HIPPA and client confidentiality laws

• Familiarity with medical terminology

• Basic computer skills to scan, organize and access electronic health records

• Must demonstrate accuracy, proficiency and efficiency with computer skills for data entry.

• Demonstrate effective people and telephone skills with the ability to communicate well with patients and other medical clinics utilizing tact and professionalism

• Advanced organization and time management skills

• Must be able to ensure that all daily actions and communication are in support of the mission, vision and values of Kaniksu Community Health

Primary Duties and Responsibilities:

• Preparing patient charts ensuring information is documented in EHR accurately.

• Ensuring that the medical records are organized, accurate and complete in a timely manner.

• Reviewing patient records, test results, and labs for relevant medical history and transcribing into data fields in the electronic patient medical records.

• Creating digital copies of paperwork and storing the records electronically

• Filing the paperwork and reports of patients quickly and accurately.

• Safeguarding patient records and ensuring that everyone complies with the HIPAA standards

• Transferring data to other clinic or facility’s main system database.

• Works cooperatively with the KCH personnel to carry out the goals and objectives of KCH according to the established policies and procedures.

• Prepares patient record information for physician referrals and communicates significant information to other health care providers as required.

• Responsible for attending and actively participating in PCMH Team Meetings, Staff Meetings, In-Service and Educational Programs.

Works with the PCMH Team to assure that all KCH patients are given top priority both personally and professionally by all medical support staff and that patients and providers needs are met through appropriate systems and the highest level of patient services.

• Responsible for providing accurate and thorough work product with attention to detail; maintains accurate maintenance on patient’s medical record according to established guidelines for electronic medical record.

• Responsible for supporting compliance with federal, state and other agency requirements. Strict adherence to confidentiality of all information related to KCH patients and organizational information.

• Responsible for managing priorities and workload, time management to ensure effective workflow in a fast paced clinic environment.

• Responsible for adherence to policies and procedures and supporting standardization and integration across the KCH organization.
